Attested copies of following certificates should be submitted along with the printed online filled application form

 

a. Pass certificate / Degree certificate

b. Marks cards of all university examinations passed (First to final year of the course)

c. Attempt Certificate issued by the Principal

d. Certificate regarding recognition of the college by the respective  Councils/regulatory bodies

e. Completion of rotating internship certificate from a recognized college where applicable

f. Registration by the respective state council wherever applicable

g. Proof of SC/ST or Category as the case may be

Upload your recent passport size color photograph.

Application fee is Rs.500/-. The fee must be paid through demand draft (DD) in favour of  “The Registrar, Rajiv Gandhi University of Health Sciences, Karnataka”, payable at Bangalore. The DD is to be enclosed along with the printed online filled application form.

Print out of online filled application form and attested copies of documents should be submitted by post or in person to The Registrar, Rajiv Gandhi University of Health Sciences, Karnataka, 4th ‘T’ block, Jayanagar, Bangalore – 560 041 on or before 30th April 2015, 5p m.

The envelope must be superscribed as “MPH (Hons.), PGET - 2015.”
